Maintenance
After printing, fill out your name and address. Specify the item in need of maintenance, i.e. Air Conditioner, or East Wall in Dining Room. Describe the problem in detail. Deposit the form in your Manager's box.
Although it is popular and convenient to verbally notify your manager of your problem, it is required that you submit a Maintenance Request Form.
